Skilled nursing, rehabilitation, dementia care. Government-operated. Park View Health Center is 5-star rated by CMS and considered a Dementia Specific center by LeadingAge Wisconsin. We are a County-owned and operated nursing care facility that was constructed in 2008, but has been providing care to Winnebago County Residents since the 1800s. Currently we provide care to 168 residents in a beautiful park setting, overlooking the Winnebago County Park. We are dedicated to providing long-term, dementia care and specialized services. Our dynamic mix of staff and contracted services has resulted in positive survey outcomes for many years.
