Skilled nursing, rehabilitation, hospice, dementia care, other. Government-operated. NDC is three separately licensed skilled nursing facilities on one campus. There are nine households across the facilities; five traditional long term care, two memory care and two short term rehabilitation. The campus has 137 licensed beds operating with a census around 120. The facility is owned and operated by the County of Dunn. The facility is also the meals on wheels provider for congregate meal sites for Dunn County.
